1. Start CDISC Compliance on Day One
The FDA requires CDISC-compliant SDTM and ADaM datasets for most NDA and BLA submissions. The mistake many sponsors make is treating CDISC mapping as a post-study activity. By that point, data collection decisions that complicate compliant mapping have already been made — variable names that don't conform, timing conventions that don't align with SDTM's visit-based structure, missing controlled terminology.
Starting CDISC compliance at the CRF design stage means your data flows naturally into compliant structures. The SDTM Implementation Guide should be on the desk of your data manager and clinical programmer from protocol finalization, not from database lock.
2. Align Your Estimand, SAP, and Analysis Before the Study Begins
The most common cause of FDA information requests is a disconnect between what the protocol said the trial was measuring and how the statistical analysis was actually conducted. Reviewers expect the estimand specification, the SAP, and the programming to form a coherent, traceable chain.
This alignment requires the biostatistician, clinical team, and regulatory strategist to be in the room together during protocol development — not sequentially reviewing each other's documents. When these conversations happen in silos, inconsistencies accumulate.
3. Invest in Your Define.xml and Reviewer's Guide
Regulatory reviewers use the define.xml and Reviewer's Guides to navigate your data package. A poorly constructed define.xml — missing variable labels, incorrect origin specifications, or broken hyperlinks to the blanks and codelists — adds friction to the review and can prompt questions that wouldn't otherwise arise.
A well-written Reviewer's Guide does more than list the datasets. It explains the structure of your data, highlights any non-standard approaches, and directs the reviewer to the key datasets and variables for the primary analyses. Think of it as an executive summary for a data-savvy audience.
4. Run Pinnacle 21 Validation Early and Often
Pinnacle 21 (formerly Pinnacle 21 Community) is the standard tool for validating CDISC compliance. FDA reviewers run it on your submission. Running it yourself — and resolving issues before submission — is not optional.
The key is running it early, not just at the end. Validation errors that surface at database lock can ripple through your entire dataset structure. Errors that surface during SDTM development, when the data is still malleable, are far easier to resolve.

5. Quality-Check Your TLFs Against the SAP, Not Just Against Themselves
The most overlooked QC step in TLF production is verifying that what was programmed matches what the SAP specified — not just that the output is internally consistent. A table that runs cleanly and matches its independent programmer verification is only half the quality check. Does the analysis method match the SAP? Does the population match the population definition in the SAP? Were the handling rules for missing data applied correctly?
This alignment check requires a biostatistician who knows the SAP well to review each table, not just a programmer comparing two sets of outputs. For pivotal studies, consider a formal SAP-to-output traceability review as a pre-submission gate.
None of these principles are novel in isolation. What makes FDA submissions succeed is treating them as an integrated system rather than a series of deliverables — where every decision from CRF design through final QC is made with the submission endpoint in mind.
